What plan review actually marks

Wall termination near windows. Outlet next to an OA intake. Roof fan too low. “Per code” with no dimensions. Show 40 IN MIN ABOVE ROOF, distance to intake, distance to property line / adjacent building, and discharge direction if you need the 5 ft exception.
